Our Encore Wish Fulfillment Program is a special initiative dedicated to bringing moments of joy and creating cherished memories for patients with a life-limiting illness and their families. Through the program, we strive to grant the wishes of our patients--whether it is a simple request like a favorite meal, a visit from a loved one, or a once-in-a-lifetime experience. These moments provide profound comfort and happiness, creating lasting memories for families during challenging times.
Specific Need:
We need your support to continue making wishes come true. Your generous donation will help us cover the cost associated with fulfilling these dreams, from transportation and accommodations to special equipment and more. Please consider donating to support our wish fulfillment program. Your contribution, no matter the size, will make a meaningful difference in the lives of our patients and their families. Together we can bring light and joy to those who need it the most.
Mission
At Hospice of Crawford County, we are committed to providing compassionate care and support to our patients and their families. Our mission is to enable patients nearing life's end to live free of pain, symptoms, and anxieties and empower them and their families to experience optimum quality of life.
Profile
Hospice of Crawford County, Inc. was incorporated in 1984 to enable patients nearing life’s end to live free of pain, symptoms, and anxieties and to empower patients and their families to experience an optimum quality of life. The hospice philosophy of care is driven by patient and family wishes and focuses on comfort and care when a cure for the disease is no longer possible. Hospice of Crawford County, Inc. first began serving our community staffed with nurses from the Visiting Nurse Association. In 1989, we became the first hospice in northwest Pennsylvania to be certified by Medicare. We maintain that certification through today. Hospice of Crawford County, Inc. is the only locally owned, non-profit hospice to service our community.
We have grown and evolved since our inception. In order to better serve both ends of the county as well as neighboring areas outside of the county, an office was opened in Titusville. In 2012, we opened Marquette Hospice House as a place where individuals with a life-limiting illness can come together with staff who will support their end-of-life choices with extraordinary care and compassion. Hospice of Crawford County, Inc. is currently staffed with Medical Directors, Nurse Practitioner, RNs, LPNs, CNAs, Social Workers, Pastoral staff, and volunteers to provide a full complement of services to our community.
Hospice of Crawford County, Inc. is a United Way Agency, a member of the National Association for Hospice and Palliative Care, and an affiliate of Meadville Medical Center.
